Microbial Keratitis and Contact Lens Wear
Contact lenses are currently worn by more than 30 million people in the United States, and many more worldwide. Dr. Schein and colleagues published the first epidemiologically rigorous studies on the most clinically important complication of contact lens wear, corneal inflammation due to microbial infection. This research, published in 1989, led directly to a change in approval for overnight wear of contact lenses by the FDA and a coincident change in professional practice. Recently, a new generation of contact lens materials with approximately 5 times the oxygen permeability of the conventional soft contact lenses has become available. Dr. Schein led a large post-market surveillance study documenting the rate of microbial keratitis associated with this technology.
Outcomes Research and Cataract SurgeryCataract is the most common cause of blindness and visual disability in the world. Cataract surgery is the most commonly performed operation in the United States and other developed and developing countries where adequate surgical resources exist. Dr. Schein and colleagues have devoted more than 15 years to the study of outcomes research related to cataract surgery. This body of work has included health services research related to the provision of cataract care, complications of surgery studied using a variety of different epidemiologic techniques, perioperative management of cataract surgical patients, and the patient perspective on disability from cataract and functional outcomes of surgery.
Refractive Error and Surgery:
We have developed and validated the Refractive Surgery and Vision Profile(RSVP), a questionnaire that assesses visual function related to refractive error and its correction. This instrument has been rigorously translated into more than 8 foreign languages and is in use in multiple practices and clinical trials internationally.
Epidemiology of Dry Eye:
Dry eye is a highly prevalent condition with significant patient impact. We performed the first population-based study of dry eye and found that symptoms of dry eye occur in approximately 14% of the elderly. In this group, the most important risk factor for dry eye was use of a number of classes of systemic medications, such as antihistamines, anticholinergics, diuretics and others.
